En 1924 un fisiólogo alemán, Otto Warburg, postuló que la transformación de una célula normal en una célula cancerosa ocurría como consecuencia de una anomalía en la cadena respiratoria mitocondrial; anomalía que llevaba, incluso en presencia de oxígeno, a que el resultante inicial del metabolismo celular de la glucosa, el ácido pirúvico, no entrase en la cadena respiratoria mitocondrial y se degradase hacia ácido láctico. algo que fisiológicamente ocurre en la gilcólisis anaerobia (por falta de las adecuadas presiones de oxígeno intracelular). Esta teoría llevó a que se le concediese el Premio Nobel en 1931, si bien estudios posteriores contradijeron este proceso conocido como Efecto Warburg. Nuevos estudios en este terreno, llevaron a que de nuevo la hipótesis de Warburg fuese aceptada como fenómeno patológico implicado en la mutación cancerosa.

Hace año y medio, aproximadamente, el oncólogo y endocrinólogo Salvador Harguindey, formado en Estados Unidos, nos propuso a un numeroso grupo de investigadores de muy diversos países, como se verá en las afiliaciones que figuran en el abstract que seguidamente reproduciremos, el hacer un estudio intenso de la Hipótesis de Warburg, cuyo resultado fue la publicación, que hoy mismo apareció en una revista científica de tan alto novel como es Seminars in Cancer Biology, analizándolo no solo en relación con la puesta en marcha del proceso tumoral, y sus posibles tratamientos a expensas de modificar el pH intracelular, si no también desde el punto de vista totalmente opuesto cual es el de las enfermedades neurodegenerativas.

El trabajo fue inmenso, y si podemos lo reproduciremos aquí, pero de momento, para quien esté interesado copiamos el abstract de la publicación. Fíjense en la cantidad de autores y la diversidad de países a los que pertenecen. ¿Se abre una nueva era para el tratamiento del cáncer y el de sus opuestas enfermedades neurodegenerativas?. En ello estamos, y somos muchos....

Abstract

During the last few years, the understanding of the dysregulated hydrogen ion dynamics and reversed proton gradient of cancer cells has resulted in a new and integral pH-centric paradigm in oncology, a translational model embracing from cancer etiopathogenesis to treatment. The abnormalities of intracellular alkalinization along with extracellular acidification of all types of solid tumors and leukemic cells have never been described in any other disease and now appear to be a specific hallmark of malignancy. As a consequence of this intracellular acid-base homeostatic failure, the attempt to induce cellular acidification using proton transport inhibitors and other intracellular acidifiers of different origins is becoming a new therapeutic concept and selective target of cancer treatment, both as a metabolic mediator of apoptosis and in the overcoming of multiple drug resistance (MDR). Importantly, there is increasing data showing that different ion channels contribute to mediate significant aspects of cancer pH regulation and etiopathogenesis. Finally, we discuss the extension of this new pH-centric oncological paradigm into the opposite metabolic and homeostatic acid-base situation found in human neurodegenerative diseases (HNDDs), which opens novel concepts in the prevention and treatment of HNDDs through the utilization of a cohort of neural and non-neural derived hormones and human growth factors.
